GAD 7 Form

This form is a self-assessment tool designed to help evaluate the presence and severity of generalized anxiety disorder (GAD) symptoms. It consists of seven questions that ask about feelings of nervousness, worry, restlessness, irritability, and fear experienced over the past two weeks. Each question is rated on a scale from 0 to 3, with the total score indicating the level of anxiety, ranging from minimal to severe. The results can help in understanding one’s mental health status and determining whether further professional evaluation may be needed.

5. Feeling so restless that it's hard to stay still
6. Becoming easily annoyed or irritable
7. Feeling fearful, as if something terrible might occur

Scoring the GAD-7 Anxiety Questionnaire

Scores are assigned as follows: 0 for "not at all," 1 for "several days," 2 for "more than half the days," and 3 for "nearly every day." The total score can range from 0 to 21.

Interpreting the GAD-7 Total Score:

  • 0–4: Minimal anxiety
  • 5–9: Mild anxiety
  • 10–14: Moderate anxiety
  • 15–21: Severe anxiety
